CRAD Request Appointment

To book an appointment with us, you can simply phone 07 5454 7844 during office hours Monday to Friday 8am to 5pm, or complete our appointment request form.

After you submit the appointment request, one of our team members will contact you during our office hours to confirm the day and time of your appointment. If we are unable to fulfill your requested date or time, we will offer the nearest available appointment. Any preparation required for your appointment will be provided, and an estimated total cost for your examination if there is a fee.

Please remember to bring your request form, Medicare card, and any pension or health care cards.
